Voice of Choices is also available by mail. To request a one year individual subscription delivered monthly to your home or post office box please fill out the form on inside back cover and mail, along with check or money order  for $22.00 made out to ‘Choices’.

Business Subscriptions
Bulk business subscriptions are available for businesses and practitioners outside our distribution area. 20 copies per month are sent first class mail to your location so you may share them with your customers. Cost is $10.00 per month or $100.00 per year.
